Admission Application

Every parent is faced with making an important decision for their own children; where they are going to attend school. With so many options it can be a difficult decision. We urge you to set up an appointment to tour our school and decide if we might be the right choice for you and your children.

We are pleased to partner with Step Up For Students, a scholarship program which provides children from low-income families an opportunity to attend private school through a scholarship. For families receiving this scholarship Masters Preparatory School does not charge additional fees for registration, books, or tuition.

Application Procedures

Admission to Masters Preparatory School is determined after a parent has submitted to following documentation:

• Completed Application Packet
• Application Fee ($100) or Scholarship Award Letter
• Florida Department of Health form 3040 (School Entry Health Exam)
• Florida Department of Health form 680 (Immunization Record)
• Birth Certificate of the Child
• State Issued ID or Passport of the parent

     And For Transfer Students (Those NOT entering Kindergarten):

• School records (transcript or report card) for all preceding grades
• MPS Release of Records form
• All disciplinary referrals or evidence of no disciplinary history

     And for students applying for Exceptional Student Education:

• An Individual Education Plan (IEP) from a public or private school (or)
• A diagnosis from a doctor or medical professional
